STEP 2
Check Eligibility
You must meet specific state and program requirements to participate in Colorado Connections Academy. Before enrolling at our online schools in Colorado, review the following eligibility requirements carefully to determine whether your student(s) is eligible to participate. Once your enrollment is confirmed, please immediately withdraw your student from their previous publicly-funded school. Students cannot be simultaneously enrolled in two public schools.
Note: Colorado Connections Academy does not accept students who are currently expelled from their local school or district. Students who have been suspended from their local school or district will be reviewed on a case-by-case basis to enroll.

- If you need a copy of your child's birth certificate, you can order it online through the Colorado Department of Public Health and Environment (CDPHE). Please visit the CDPHE's vital records website for more information.

Child’s Proof of Immunization
All Colorado Connections Academy students must provide evidence of their immunizations on one of the following:
- Colorado Department of Public Health and Environment Certificate of Immunization form
- Immunization booklet with all or most recent doses signed/stamped
- Official school record signed/stamped by a healthcare provider

Not Sure What Immunizations Are Required?
- For a complete listing of immunizations required by the Colorado Department of Health and Environment, view the School Immunization Requirements webpage.
Where Can I Get Immunizations for My Student?
Immunizations can be obtained from doctor’s offices, clinics, or hospitals. Pediatricians and family doctors or their nurses or medical assistants can give your student the shots he or she needs to meet the requirements for school enrollment. In addition, some large chain pharmacies offer immunizations for older children and adults.
What if I Need Assistance Paying for Vaccines?
If you need assistance paying for shots/vaccinations, you may be able to get free or reduced immunizations through this program:
- Vaccines for Children (VFC) program is a federally funded program that provides vaccines at no cost to children who might not otherwise be vaccinated because of inability to pay. Children who are eligible for VFC vaccines are entitled to receive pediatric vaccines that are recommended by the Advisory Committee on Immunization Practices.
- The Vaccines for Children (VFC) helps to further the VFC program’s initiative for Colorado families. VFC provides free vaccines to virtually all local public health agencies (LPHA) in Colorado. Each county has an LPHA or clinic that serves as a "safety net" for low-cost vaccines. The vaccines are available for all children who are eligible for VFC and for children who are underinsured (i.e., high copay, deductible). Contact The Vaccines for Children Program to learn more about school immunization requirements and VFC providers in your area.
Immunization Exemptions
Colorado allows for medical, personal or religious exemptions, however medical exemptions must be signed by a health care provider.
- Religious or Personal Exemption: A parent or guardian must complete, sign and date the Colorado Department of Health and Environment Immunization Non-medical Exemption Form, which states that the student or the parent/guardian is an adherent to a personal or religious belief opposed to immunizations.
- Medical Exemption: A physician must complete, sign and date the Colorado Department of Health and Environment Immunization Medical Exemption Form, which states that the physical condition of the student is such that immunization would endanger life or health or that it is medically contraindicated due to other medical conditions.
